Physical Therapist
Job Summary
A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.
Qualifications
- A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
- An active license to practice Physical Therapy in FL
- A minimum of one (1) year experience as a Physical Therapist, preferably in the homecare setting
- Must have criminal background check
- Must have current CPR certification
Occupational Therapist
Job Summary
An Occupational Therapist provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. If Nursing has opened the case, Occupational Therapy is a secondary discipline and therefore works collaboratively and professionally with the RN case manager to provide necessary services.
Qualifications
- Will be a graduate of an OT Program approved by the American OT Association or Council on Medical Education
- A current license in FL
- A minimum of one (1) year experience as a OT, preferably in the homecare setting
- Must have a criminal background check
- Must have current CPR certification
Licensed Practical Nurse
Job Summary
A Licensed Practical Nurse (LPN) provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates patients and their families according to the established professional plan of care.
Qualifications
- A graduate of an LPN program
- An active license to practice professional practical nursing in FL
- A minimum of one (1) year experience as a professional nurse, preferably in the homecare setting
- Criminal Background check
- Current CPR certification
Registered Nurse
Job Summary
A nurse currently licensed who provides professional services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families.
Qualifications
- Will hold an active license to practice professional nursing in FL
- A minimum of one (1) year experience as a professional nurse, preferably in home care
- Preferably has completed a BA degree program approved by the National League for Nursing for Public Health Nurses
- Must have a criminal background check
- Must have current CPR certification
Home Health Aide / Certified Nursing Assistant
Job Summary
The HHA/CNA provide personal care and related services in the patient’s residence. She/he functions under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor.
HHA Qualifications
- A High school diploma or equivalent
- Successful completion of an at least a 40 hour training, that included the topics outlined in FL Statutes 400.497(1) F.S OR Successful completion of the AHCA HHA Competency Test (AHCA form 3110-1007), in lieu of the 40 hour training
- Prefer one (1) year experience
- Must have a criminal background check
- Current CPR certification
CNA Qualifications
- A High school diploma or equivalent
- Successful completion of a CNA training and presentation of a copy of the certificate of completion
- Prefer one (1) year experience
